Transitioning to Adult Care
Transitioning from pediatric to adult care is a significant milestone for families caring for a child with complex medical needs, including those receiving pediatric palliative care. This stage often brings new systems, eligibility requirements, and decisions that can feel overwhelming.
Working in partnership with the Roger Neilson Children’s Hospice Family Advisory Committee, we hear that families need clear, reliable information to navigate financial support, health services, legal considerations, and community resources.
With this in mind, we have collected a wide range of resources from organizations, government programs, and advocacy networks to create a guide that focuses on the topics that matter most to families—such as planning for future care, accessing financial assistance, finding respite options, and understanding decision-making supports.
While no single resource can cover every situation, this page brings together practical tools and links to help caregivers feel informed and empowered as they guide their loved one into adult care.
